Good Faith Estimate Notice

Denise A. Lucas, LPC
Effective Date: 07/02/2026

Your Right to a Good Faith Estimate of Expected Charges

Under the No Surprises Act (2022), health care providers are required to give patients who are uninsured, or who are not planning to use insurance to pay for their care, an estimate of expected charges before treatment begins.

Who This Applies To

This right applies to you if:

You do not have insurance, or
You have insurance but choose not to use it to pay for your care (private pay)

If you are using your insurance benefits to pay for services, your costs will instead be determined by your insurance plan (copay, coinsurance, deductible, etc.), and a Good Faith Estimate is not required in the same way — please contact your insurance provider directly for questions about your specific plan costs.

What a Good Faith Estimate Includes

If you are uninsured or paying privately, you have the right to receive a Good Faith Estimate in writing that includes:

The expected cost of your therapy sessions
The frequency of sessions being recommended
Any other related charges you may reasonably expect

How to Request Your Estimate

A Good Faith Estimate will be provided to you before your first scheduled appointment, or you may request one at any time by contacting:

Phone: 503-782-9950

You have the right to request a new Good Faith Estimate at any point during care if your needs or treatment plan change.

What to Do If You Are Billed More Than Your Estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the charge.

You may start a dispute with the U.S. Department of Health and Human Services (HHS) Patient-Provider Dispute Resolution process. Starting a dispute will not affect the quality of care you receive from this practice.

To learn more, or to start a dispute, visit www.cms.gov/nosurprises, or call 1-800-985-3059.

Keep a copy of your Good Faith Estimate for your records. For questions about this notice or about your specific estimate, please contact Denise A. Lucas, LPC directly using the information above.
