 A free 15-minute consultation is scheduled to provide you and Denise the opportunity to meet and discuss if services are appropriate for you. It also provides you with time to ask any questions you may have as well. Because time is limited, you will be emailed an online questionnaire and asked to complete it and email back 24 hours before the consultation begins. If the questionnaire is not received 24 hours prior to the consultation, the appointment will be rescheduled.

How to get care – Call 503–782-5590 or email [email protected] to set up an initial consultation. You will be sent an online consultation form to complete and returned 24 hours prior to your appointment time. If the form is not completed 24 hours prior, your appointment will need to be rescheduled.
