I am currently accepting telehealth-only patients

 A free 15-minute consultation is scheduled to provide you and Denise the opportunity to meet and discuss if services are appropriate for you. It also provides you with time to ask any questions you may have. Because time is limited, you will be emailed an online questionnaire and asked to complete it and email it back 24 hours before the consultation begins. If the questionnaire is not received 24 hours before the consultation, the appointment will be rescheduled.
